Note: If you cannot see the entire application form below, please go here to fill out the dog application or here to fill out the cat application. Our forms work best if filled out on a computer instead of a smartphone/tablet.
If you have filled out an adoption application in the past year, you do not need to fill out another one. Please email adopt@pawsitivematch.org or your previous adoption processor to add a new dog or cat to your list or to reactivate your application.
Dog Adoption Application
IMPORTANT: Your application is vital to our rescue organization so we strive to reply to each one within 48-72 hours. If you have not been contacted within that time, please email adopt@pawsitivematch.org to confirm that we have received your application.

NOTE: If you rent your home or own a condo in a building governed by a condo board, Pawsitive Match requires written approval prior to adoption. To avoid delays in the processing of an adoption, please have your landlord or condo board fill out and submit the landlord approval form found on our Adoption Process page as soon as possible. Completed forms can be emailed to: adopt@pawsitivematch.org

Dog Adoption Application
Pawsitive Match Rescue Foundation charges an adoption fee. These fees help to cover expenses incurred by the rescue. All animals adopted through Pawsitive Match will be spayed/neutered and up-to-date on certain vaccines. If they are too young to be spayed or neutered at the time of adoption, we will have the surgery performed at one of our partner veterinary clinics in Calgary at no cost to the adopter.
By electronically signing this application, you confirm that you are in agreement with the following statements:
- I hereby certify that all statements are true.
- Should there be any changes to the information provided, I will notify Pawsitive Match Rescue Foundation as soon as possible.
- There has been no event or information not disclosed in writing to Pawsitive Match Rescue Foundation which could reasonably be expected to have an adverse effect on the health, comfort, safety, or well-being of the rescued animal.
- I am aware this application must be approved before an adoption can take place.
- I have read the application and understand its meaning and agree to its terms.
- By submitting this application to Pawsitive Match Rescue Foundation, I agree and consent to the collection, use and transfer of my personal information contained in this application for the purposes of processing my application.
Please note that our applications work best when filled out on a computer and sometimes will not come through when filled out on a mobile device. You will receive a confirmation email with a copy of your answers if you have submitted this form successfully. If this email does not come through, please check your spam/junk mail folder. If you have not heard from a volunteer within 48 hours of applying, please email adopt@pawsitivematch.org

Landlord Pet Approval Form

Please note that completed landlord pet approval forms must be submitted to adopt@pawsitivematch.org in order to start the application approval process. If we have not received your form, your application will be put on hold until it is received.
